Forty-year old “colored” female born in Isle of Wight, Virginia. Had resided in Portsmouth, Virginia for 24 years. Residence/place of death was 323 Washington Street. Died November 29, 1890. Parents were Thos & L. Baker. Occupation, Minister of Baptist Church. Found dead on the floor of her residence. Buried at Wilson Cemetery on December 3, 1890. Undertaker was Samuel Fisher.

Mother was Lucy Lewis. Occupation, Minister of Baptist Church. Cause of death was cerebral hemorrhage. Buried at Wilson Cemetery on December 3, 1890. Undertaker was Samuel Fisher.

“W.H. Lewis … [22 years old] … he is the legal guardian … soldier died Nov 29 / 90 … wife died Aug 5 / 90 … That the father was married under the name of Ashley Lewis to Josephine Baker
“Also personally appeared, Jacob Gaskins, residing at Portsmouth, Va. and Enos Dennis, residing at Portsmouth, Va. … they knew [Ashley Lewis] for 22 years and 15 years, respectively…”

“I, C.T. Phillips, Clerk of the Court of Hustings for the City of Portsmouth do certify that it appears from the records in the clerk’s office of said court that Sarah V. Lewis was born at Portsmouth, Va. May 20th, 1876; that Thomas Lewis was born at the same place September 6, 1882; that Leon Fayette Lewis was born at the same place November 18th, 1884; and that all the above named were the children of Ashley H. and Josephine Lewis.”

She now reasserts that June 25, 1887 was the date of birth of John R. and she finds her memory verified by the family record as contained in the family Bible, which entry escpaed her notice when she made the formal affidavit.
